Has your GP or specialist given you a referral to Sleep and Lung Care? Upload it here and our reception team will phone you to arrange your first appointment at Epping, Brunswick, Doncaster East, Braybrook and Kilmore. First appointments are always in person, so that examination and any tests can happen. New appointments are usually available within about 3 weeks.
Upload a PDF or a clear photo of your referral (JPG, PNG or HEIC), up to 10 MB per file. Include every page. If your doctor has already sent the referral, please call reception on (03) 9422 0077 rather than uploading another copy.
What happens next
- We check the referral and any test results it includes.
- Reception phones you within two business days to book a time and clinic that suit you.
- Bring your Medicare card, medication list and any X-ray or CT reports to your appointment.
